Appendix C — Decision tree for determining a VCCP

(Reproduced in full in Step 10 of this guide.)

Summary of the flow:

  1. Q1: Is V ≥ 5.0? → NO: document and monitor. YES: continue.
  2. Q2: Is there a specific mitigation measure in place? → NO: a VCCP is required. YES: continue.
  3. Q3: Is the measure verifiable (measurable criteria + monitoring)? → NO: a VCCP is required. YES: continue.
  4. Q4: Has the measure been shown to be effective (12 months of evidence)? → NO: a VCCP is required. YES: it is a VCCP that is already managed.

Special rule: If V ≥ 10.0, the VCCP is established automatically (skip to Step 11).
